Clinical examples tracheal stenosis

Case 3. Anterior and posterior subglottic stenosis.

 

63-year-old female presenting with circumferential subglottic stenosis after intubation during a 2 weeks period. There is ankylosis of the arytenoids. Stenosis ends at lower margin of the cricoid cartilage.

a. Axial CT scan of stenosis.

b. Schematic presentation of reconstruction. The larynx is incised in the anterior and posterior midline. A rib cartilage graft (1) is placed posteriorly. The mucosa-lined fascia (2) will be inserted anteriorly. A silicone tracheal stent (3) is tailored to support the reconstruction during 1 month.

c. Situation after stent removal.

       1. Axial CT scan at level of arytenoids.
       2. Axial CT scan at subglottic level.

       The rib cartilage is visible posteriorly; the mucosa-lined fascia is visible anteriorly.
